Our aim is to provide continuing personal care to patients on our list and to offer the comprehensive range of services expected of a modern General Practice.
Much greater importance is now attached to prevention of illness and adopting healthy lifestyles and we encourage our patients to attend us for help and advice when well.
We appreciate the need to provide for consultations outside the normal working day, but as these appointments are limited, it would be helpful if patients could attend daytime surgeries, whenever possible.
Evening GP appointments
For patients registered at participating surgeries in Rochford, Rayleigh, Benfleet & Canvey.
Monday to Friday 6:30pm-8pm
This is not a walk in service – contact your own surgery to book an appointment.
New members of the team
There are now Advanced Nurse Practitioners, Emergency Care Practitioners and Pharmacists working with your GP. Your query may be directed to them if they are best suited to meet your care needs.
Please ask at your GP surgery if you would like more information.
One form for each person registering will need to be completed, signed and dated. You are requested to book a Health Check with the Nurse, on registering.
The boundary for our Surgery, to take on new patients, operates over the whole of Canvey Island.
We are proud of our premises which have been purposely adapted and converted, but retains the character and charm of the original building. We are able to provide facilities for disabled patients, as well as purpose built consulting rooms and a treatment room equipped to perform minor surgical procedures.
Change of Address
It is very important that we hold a correct address and telephone number for you. If either of these change please notify us either by completing a form at the surgery reception desk, advising by telephone on 01268 510520 or log in to access the patient area to change your address online.
NHS Friends and Family Test
The NHS want you to have the best possible experience of care. The NHS Friends and Family Test is a way of gathering your feedback, so we can continually review our service.
GP Patient Survey
The GP Patient Survey is an independent survey run by Ipsos MORI on behalf of NHS England. The survey is sent out to over a million people across the UK. The results show how people feel about their GP practice.
